Title: Source Code cleanup

I have been tidying up the source code recently and the results are attached. :)

FBA now compiles completely cleanly with GCC 3.4.5. It also compiles with GCC 4.3.3, with about 16 million warnings. :)

I have also successfully compiled completely cleanly using the latest Windows Server 2008 SDK 6.1, and the August 2007 DirectX SDK which is the last to support Direct3D 7. I'll probably change to the latest DirectX SDK and add back support for Direct3D 7 using the August 2007 files.
